Short answer
Apply at the ward office of your permanent address with an application, the citizenship copies of everyone named, documents that show the relationship (birth or marriage registrations) and two photos of each person. If the relationship isn't clear from documents, the ward holds a local verification with witnesses. Most wards issue it the same day, in Nepali or English. The fee is set by each municipality's finance act.

What it's used for
Inheritance of land and bank accounts, insurance claims, student and family visas abroad, the citizenship of family members and pension or social security claims.
Fees
Each municipality sets its fee in its yearly finance act. English versions usually cost more than Nepali ones. Ask your ward or check the municipality's citizen charter.
Common problems
I need it in English for a visa.
Ask the ward for the English version. Many municipalities issue both and charge more for English. For use abroad, the certificate usually also needs attestation by the Ministry of Foreign Affairs' Department of Consular Services.
A family member lives in another municipality.
Bring their migration certificate if their citizenship is from outside the municipality. The ward issuing the certificate needs to see why they're included.
Can I include a child who doesn't have citizenship?
Yes. Use the child's birth registration certificate instead of a citizenship.
It's for an inheritance after a death.
Bring the death registration certificate too. The relationship certificate should list every heir, because the Land Revenue Office and banks rely on it.
