Short answer
Apply for a duplicate (प्रतिलिपि) at the District Administration Office that issued your original. Bring a ward recommendation on the set form, two photos and anything showing your certificate number and issue date, such as an old photocopy. The fee is a Rs 13 stamp and the copy is issued the same day once your papers are complete. It keeps your original citizenship number.

Where to go
The duplicate (प्रतिलिपि) section of the District Administration Office. The quickest route is the office that issued your original, because it holds your record. If you've moved, you can apply in your new district with the extra documents in the questions below, but that office has to confirm your record with the issuing district first.
What you get
The duplicate carries the same citizenship number and details as the original, so your National ID, PAN, passport and bank accounts stay linked. If you find the old certificate later, keep using the duplicate.
Do you need a police report?
No. A police report or newspaper notice isn't on the list of documents in the district charters we checked.
Common problems
I don't have a photocopy and don't know my certificate number.
Try places that kept a copy: your bank, your employer, your school or college, the ward office or any office where you applied for a passport, PAN or licence. Your National ID record, if you have one, also carries the number. Without the number the office has to search its register, which takes longer.
I now live in a different district from where my citizenship was issued.
You can apply where you live now. DAO Kalikot lists the extra documents for this case: your migration certificate, the land ownership certificate for your permanent home and confirmation from the issuing district that its record matches. The copy is only issued once that confirmation arrives, so expect it to take longer.
I'm married and want my husband's name on the copy.
Bring your marriage registration (original and copy), and your husband has to come in person with his original citizenship to verify you. If your citizenship came from another district, the record check applies too.
My certificate is torn or the photo is unrecognisable.
Apply for a copy the same way and bring the damaged certificate instead of a photocopy.
I'm a government employee's family member.
Add a recommendation letter from the employee's office and a copy of their staff ID.
